Forms
*All forms are due during registration dates above. Forms can be downloaded from this site or will be provided at registration. If you plan to complete the forms onsite be prepared to provide all medical information including doctor's name, contact info and insurance policy number, etc. It's very IMPORTANT to know that both guardians are required to sign the Parental Consent form.
*If all forms cannot be turned in during registration you will be given an envelope so after you do complete them you can deliver them to the PTSA drop box in the office by due date!!
Payment
*A deposit of $50 is due the night of registration to hold your child's spot so we can provide the camp with accurate numbers of campers from our school.
*Full payment (minus the $50 deposit) is due no later than 28 Jan 2017 to the camp leaders Tom O'Malley or Samantha Boice. If you are unable to make the full payment by 5 Feb 2017 please contact Tom and Samantha as early as possible so you can work out an alternate plan.
